Save Valuable Time with this Consultation Checklist

In order to make most out of your time with Dr. Bastuba, we have created a checklist to help you prepare for your first consultation. Following the steps below will help you have a smooth consultation.

Checklist for Your First Consultation

  1. Check Your Health Insurance
    Contact your medical insurance carrier and find out if you need a referral or an authorization. If applicable, please make sure your primary care physician has handled this requirement before your appointment. If possible, investigate if you have any health benefits for infertility, sexual dysfunction or urology treatments.

  2. Learn About Financial Options
    Please review our payment policy and financing options before your visit.

  3. Make an Appointment
    Call the office at 619-286-3520 and make an appointment for an initial consultation or request a consultation through our secure online contact form.

  4. Download New Patient Forms
    Print the forms from our patient download center and fill them out beforehand to save time. Please have them filled out at the start of your appointment.

  5. Obtain Medical Records
    If applicable, request from your physician copies of any previous male fertility evaluations, especially if you had a semen analysis.

  6. What to Bring
    At your first appointment, please bring the completed & signed new patient forms, your insurance card and any medical records related to fertility treatment, especially OP reports, semen analysis and radiology reports. Please note that the fee for your first consultation for male fertility is $300.
